Forward Horizons Inc hereby provides notice that it intends to apply to the Wisconsin Department of Transportation program for the following project under Section 5310 to serve seniors and individuals with disabilities in Ashland & Bayfield counties: 1 minivan, rear-entry, seating 3/2. Estimated total cost: $79,488. Estimated 20% match cost: $16,839. Formal comments or requests for additional information must be received in writing or by email within the period of 14 days after publication.

The Children's Long-Term Support (CLTS) Program is a Home and Community-Based Service that supports and services for children who have substantial limitations in their daily activities and need support to remain in their home or community.
